Medicare requires certain information to be on file in order for us to file for reimbursement for your supplies. USE OUR CHECKLIST: 1. Call Leon or Bill for correct Mediacre form. 2. Fill out Medicare form. 3. Have your physician sign it. 4. YOU CANNOT FAX THE FORM. It has to be mailed to: MEDIC EQUIP. COM
5. Include a copy of your Medicare card, front and back. 6. Include a copy of your secondary insurance card front and back.
